Upper Arch Cerec Porcelain Reconstruction
Our patient, a professional religion educator and long standing (10+ years) patient of record, presented in our office to discuss some serious dentistry.
Reparative Dentistry
Over the years she tended to request reparative dentistry, as many of us do, for attending to treatment concerns and needs affecting us "at the moment."
She was at the point now where she wasn't too pleased with the condition of her teeth and, since she enjoyed a high visibility career as an educator, was keenly interested in acquiring a cosmetic appearance that would have a dramatic effect that she could actually feel.
Nothing short really, in her terms, of a cosmetic, yet very functional makeover.
Reparative Dentistry, as we all know, is great for getting things done efficiently. As dental technologies and treatment products evolve (at a rapid pace) a collection of individualized treatments can create dentition that is made up of different types of materials that end up feeling unbalanced for some patients. Teeth treated in a haphazard sequence without regard to an overall biting scheme, tend to breakdown faster than teeth treated at the same time with total control over occlusal schemes (a comfortable bite).

Our pre-treatment before pictures show the degree of cosmetic need that patient had for her upper arch. A large gap (diastema), uneven gum line (gingival tissues), lack of symmetry between tooth structures and some minor whitening issues.
Due to normal wear and tear of tooth structures, our patient was also beginning to develop a collapsed bite which can produce the effect of appearing prematurely older (in severe cases-known as granny mouth). Our patient wanted her new teeth to show more fully when she smiled, as they once did.
Dr. Richard presented a number of treatment plan choices that would address the patient's goals in several different ways. Her goal was quite definite: she wanted great looking teeth that would have the longest endurance possible, coupled with a cosmetic result that could undeniably produce that certain makeover feeling she wanted.
Our patient selected the use of custom premium porcelain that would be handcrafted to her specifications. With the use of diagnostic wax-up models developed by Dr. Richard our patient participated in the re-sculpturing of her upper arch.
Premium Materials begets Premium Results
She selected several popular technologies that would be used to create a series of 10 porcelain crowns, each having properties of natural fluoresence and translucency that mimics Mother Nature herself (these properties overcome the common flat white appearance of common or average dental crowns).
Dr. Richard used specialized laboratory fabricated Cerec porcelain with IPS Design Porcelain to deliver the precise natural look and feel the patient was seeking. Long term endurance was a "given" with this degree of craftsmanship. She felt secure in knowing her crowns would last for years to come.
Dr. Richard maintained a watchful eye on bite and occlusal issues while our patient remained focused on her desired makeover result. His use of an articulator (a standard for reconstructive treatment plans in our office) enabled him to recreate the bite and occlusal dynamics that would establish the larger smile and normalized bite.

Our 2 post treatment after photos clearly reveal an upper arch that appears and feels natural. Notice how the upper lip now moves up when smiling... to reveal more of her teeth. This is possible only through underlying support that only properly designed and fitted restorations can provide.
Post Treatment Patient Comments
Our patient was thrilled with the outcome. Only a few people are aware that her great looks were created by her and Dr. Richard.
Her only complaint was that she wished she had done it years earlier. Smiling broadly and naturally came easy for her now. She also reported that her speech had improved (important for a professional educator).
